Become a Patient
**November 2023 Update: We have openings for new prenatal patients at our Somersworth location (Goodwin Community Health); delivery would be at Wentworth-Douglass Hospital. We do not currently have openings for new prenatal patients at our Portsmouth location, Families First.**
Please follow the instructions below to become a prenatal patient at one of our health centers in the NH Seacoast region.
To become a Prenatal Patient (5 options)
- Call Goodwin Community Health (603) 749-2346) and choose ‘Patient Services’ and then ‘Prenatal Department,’ and we will get you set up with care.
OR - Stop by Goodwin Community Health in Somersworth or Families First in Portsmouth during business hours (check our hours) to pick one up.
OR - Download the prenatal intake packet and print it out. Also, if you are not already established as a Primary Care Patient at Goodwin Community Health or Families First, please download, print and fill out these additional forms.

- Complete the forms during your intake appointment.
Returning the Intake Packet, and What Happens Next
- Mail or bring the form to the practice where you would like to receive care:

What's Next
One of our social workers will call you to schedule your appointment. Let them know if you will need translation / interpretation.
- Arrive 5-10 minutes early to finalize paperwork.
- Bring your forms if you haven’t mailed them in.
- Bring a list of the medications you take, or the actual medications.
- Bring your insurance co-payment or sliding-fee scale payment. One of our Patient Advocates will let you know the amount of your sliding-fee scale payment if you qualify for the scale.
