**November 2023 Update: We have openings for new prenatal patients at our Somersworth location (Goodwin Community Health); delivery would be at Wentworth-Douglass Hospital. We do not currently have openings for new prenatal patients at our Portsmouth location, Families First.**

Please follow the instructions below to become a prenatal patient at one of our health centers in the NH Seacoast region.

To become a Prenatal Patient (5 options)

  • Mail or bring the form to the practice where you would like to receive care:

What's Next

One of our social workers will call you to schedule your appointment. Let them know if you will need translation / interpretation.

  • Arrive 5-10 minutes early to finalize paperwork.
  • Bring your forms if you haven’t mailed them in.
  • Bring a list of the medications you take, or the actual medications.
  • Bring your insurance co-payment or sliding-fee scale payment. One of our Patient Advocates will let you know the amount of your sliding-fee scale payment if you qualify for the scale.
