Becoming a New Patient (Primary Care, Pediatric Care)

We are currently accepting new patients for Prenatal Care, Pediatric Care and Adult Primary Care at both our Portsmouth and Somersworth locations. Getting Started as a New Patient

Step 1 Register

You have a choice of a few ways to do this:

Choice 1 : Fill in online form (the easiest way!)

Choice 2: Print the form, fill it out and return it

Click here to open the intake packet

You may print it out and fill it in by hand, or fill it in online and then print it out. See next sections for instructions on how to return the forms.

Choice 3: Ask us to mail the forms to you

Please call 603-749-2346 ext. 2535 or send an email to tbest@goodwinch.org to request a packet. 

You may drop them off at Families First in Portsmouth or Goodwin Community Health in Somersworth, or mail them to:

Please do not use these forms if you are already a patient at Families First or Goodwin. Instead, find the form you need in the Useful Forms section of the  Patient Forms & Info page.

After filling in the forms, mail them to:

As you are filling out the Informed Consent (pages 3-4 of the intake paperwork), you will be asked to acknowledge that you have been given access to the following documents. Please click the links below to read them. 

Step 2 We Will Contact You

For Primary Care (adult and pediatric): It may take up to a month for adult primary care applications to be processed pending appointment availability and receipt of previous medical records. (Pediatric appointments are available sooner.) Please be patient, and feel free to call us at (603) 749-2346 ext. 2535 or use this form to send a message if you have questions.

Step 3 Go to Your Appointment

Please arrive at your first appointment 15 minutes before the scheduled time so you can fill out the intake paperwork.

bring to your first appointment:

  • A list of the medications you take, or the actual medications
  • Your insurance cards, co-payment or sliding-fee-scale payment

If you ever find that you can’t keep an appointment or will be late, please call to let us know.
