Getting Started as a New Primary-Care Patient

Step 1 Register

Please note: As of Spring 2023, due to the large number of people submitting the paperwork to become new patients, it may take us up to a month to process your application. Please be patient, and feel free to call us at (603) 749-2346 ext. 2752 if you have questions. 

There are three ways you can complete the registration paperwork. Please choose from the options below. All forms must be completed before your first appointment can be scheduled.

Step 2 Wait for Us to Contact You to Schedule an Appointment

Step 3 Go to Your Appointment

Please bring to your first appointment:

  • A list of the medications you take, or the actual medications
  • Your insurance cards, co-payment or sliding-fee-scale payment

If you ever find that you can’t keep an appointment or will be late, please call to let us know.
