Accepting New Pediatric and Prenatal Patients

Please read this information about our capacity to schedule new patients as of Fall 2023.

  • New pediatric patients are being scheduled at both our Portsmouth and Somersworth locations. To see how to register your child as a pediatric patient, scroll down a little bit. 
  • New adult primary-care patients are being scheduled in Somersworth; first appointments are usually available about 4-6 weeks after the intake paperwork is complete. Our Portsmouth location is not currently scheduling appointments for new patients. 
  • Adults who would like to become patients in Portsmouth may start by having a New Patient appointment with one of our providers in Somersworth. At the end of that appointment, ask us about availability to be seen in Portsmouth in the future.
  • It may take up to a month for your application to be processed

Getting Started as a New Patient

Step 1 Register

You have a choice of 4 ways to obtain a registration packet for new pediatric patients:

  1. Download the Pediatric New Patient packet hereThen print it out, fill it out, and drop it off at one of our health centers or mail it to the address below. OR …
  2. Stop by Goodwin Community Health in Somersworth or Families First in Portsmouth during business hours (check our hours). OR …
  3. Call our New Patient Coordinator at (603) 749-2346 ext. 2752.
  4. Use this form to ask us to mail paperwork to you.

A single “Release of Medical Information” form is included in the main registration packet. If you need additional copies of this form, see “Need additional ‘Release of Medical Information’ forms” section just below.

As of Fall 2023, new adult primary-care patients are being scheduled in Somersworth only (not in Portsmouth). More information about this is above.

Step 2 Wait for Us to Contact You to Schedule an Appointment

Due to the large number of people submitting the paperwork to become new patients currently (Fall 2023)  it may take us up to a month to process your application. Please be patient, and feel free to call us at (603) 749-2346 ext. 2752 if you have questions.

Step 3 Go to Your Appointment

Please bring to your first appointment:

  • A list of the medications you take, or the actual medications
  • Your insurance cards, co-payment or sliding-fee-scale payment

If you ever find that you can’t keep an appointment or will be late, please call to let us know.
