How to Access the Patient Portal
Ask the office for your Personal Identification Number (PIN) if you do not have it yet. To protect your personal health information, this must be done in person.
Go to the Portal login page for your health center:
See the “Patient Information – Portal Welcome Handout” that you received when you were given your PIN at the office. It includes your PIN and instructions on how to create your account. If you lose the handout, call your health center immediately.
Create your account. You will only need to do this the first time you use the portal.
- FIRST, create your account, including choosing your password. (Note: Your password is not the same as your PIN.)
- THEN, you will be prompted to add the PIN that you received in person and that is written on the handout.
Log in to the Portal using the login box on your computer screen or the Log In menu on your smartphone.

The Patient Portal is a secure website intended to communicate confidential medical information to the patient. Anyone who misrepresents themselves as the patient to gain access to this information is subject to punishment by applicable law. Please keep your password secure as this is equivalent to your signature and allows access to your medical record. Greater Seacoast Community Health cannot assume responsibility for stolen or borrowed access to your account. Please let us know if you would like your account disabled for any reason.
